What we do with your details
Short version: we use what you tell us to arrange your ride, and nothing else. We do not sell it, and we do not share it with anyone who is not helping get you to your appointment.
What we collect
When you send the booking form or call us, we take the rider's name, a phone number, and an email address if you give one. We take the pickup address, the destination, and the date and time of the appointment. We also take whatever you write in the notes box, which is often something about mobility, a caregiver riding along, or a preferred language.
We ask how the ride is being paid for, and if it is Medical Assistance we may record a member number and a trip authorization number, because the payer requires them.
Why we collect it
To arrange the ride and to be able to reach you about it. A dispatcher needs to know where to send a driver, when, and whether anything about the trip needs planning. That is the whole purpose.
Who can see it
The people at Bloomlake who schedule and drive the rides. The driver assigned to your trip sees what they need to collect you and get you there.
Your details are stored in the customer system we use to run the business, which is provided by a company called HighLevel. Storing records with a software provider is normal for any transport company, and they are held under that provider's terms rather than published anywhere.
If your ride is billed to Medical Assistance or to a health plan, we share what that payer requires in order to be paid for the trip. If a clinic or a case manager arranged the ride on your behalf, we may confirm details of the trip with them, because they arranged it.
What we never do
We do not sell your information. We do not rent or trade it. We do not hand it to advertisers, and we do not use it to market anything unrelated to your transportation.
How long we keep it
We keep ride records for as long as we need them to run the service and to satisfy the record keeping that Minnesota and the payers require of a transportation provider.
Asking us to delete your details
Call (612) 314-3637 or email info@bloomlaketrans.com and ask. We will remove what we are not required to keep, and we will tell you plainly if something has to stay on file for a payer or a regulator.
You can also ask us what we hold about you, and we will tell you.
A note about the form
Sending the booking form does not confirm a ride. It reaches our scheduling team, and a person calls you back to agree the details. If you would rather not put anything in writing at all, call us instead and we will take it over the phone.
